By default, Google Forms don’t collect email addresses. This is useful because people can respond anonymously and because people don’t need a Google account to fill out your form.
If possible, work with a graphic designer to customize an initial Google Form. Then, share that form so that “everyone in the organization” may access it — and reuse the customizations.
