The Washoe County Regional Medical Examiner’s office is releasing new details about an officer involved shooting that occurred at the VA hospital in Reno. They say 58-year-old Jeffrey Chapman of Reno was pronounced dead at the scene.

The Washoe County Regional Medical Examiner’s Office has identified the man who was fatally shot by law enforcement at Reno's VA hospital early Wednesday as Jeffrey Chapman, 58, of Reno.

The second OIS occurred Wednesday morning around 2:30 am at the VA hospital in Reno. The Washoe County Medical Examiner’s Office says 58-year-old Jeffrey Chapman reportedly entered the hospital’s ER brandishing a weapon and was shot dead by law enforcement after charging them with that weapon.
